If you are in need of a birth certificate and were born in Ohio, all you need to do is show up and we can help. We'll help fill out the paperwork and provide a voucher that is good for 48 hours.
If you bring the paperwork required for an ID, we will check it and give you a voucher good for 48 hours. What do you need for an ID? Proof of the five following elements:

- Full legal name
- Date of birth
- Legal presence in the U.S.
- Social Security Number
- Ohio street address. Two separate documents with your Ohio street address
- Proof of name change (if you've changed your name)
